수산식품산업의 육성 및 지원에 관한 법률 시행규칙 제36조(원산지인증서의 발급)

① 우수수산식품등인증기관의 장은 제35조에 따른 심사 결과 수산가공식품 또는 음식점등이 영 제34조 및 이 규칙 제37조에 따른 원산지인증의 기준에 적합한 경우 별지 제33호서식의 수산가공식품 원산지인증서 또는 별지 제34호서식의 음식점등 원산지인증서를 신청인에게 내주어야 한다.

② 제1항에 따른 원산지인증서가 분실ㆍ훼손되거나 기재사항 중 업체명 또는 대표자명이 변경되어 원산지인증서를 재발급받으려는 자는 별지 제35호서식의 원산지인증서 재발급 신청서에 다음 각 호의 서류를 첨부하여 우수수산식품등인증기관의 장에게 제출해야 한다.
