Enforcement Rule of the National Health Insurance Act, Article 4 (Reports on Acquisition, Change, and Loss of Subscriber Status)

① 세대주는 그 세대의 구성원이 법 제6조제3항ㆍ제8조제1항 및 제9조제1항에 따라 지역가입자의 자격을 취득한 경우 또는 지역가입자로 자격이 변동된 경우에는 별지 제5호서식의 지역가입자 자격 취득ㆍ변동 신고서에 보험료 감면 증명자료를 첨부(법 제74조 및 제75조에 따라 보험료가 면제되거나 일부를 경감받는 사람만 해당하며, 공단이 법 제96조에 따라 국가 등으로부터 제공받은 자료로 보험료 감면 대상자임을 확인할 수 있는 경우에는 첨부하지 않는다)해 공단에 제출해야 한다. 다만, 제4항제2호에 따라 사용자가 별지 제8호서식의 직장가입자 자격상실 신고서를 공단에 제출한 경우에는 별지 제5호서식의 지역가입자자격취득ㆍ변동신고서를 제출한 것으로 본다. <개정 2018.12.18>

② 사용자는 법 제6조제2항ㆍ제4항, 제8조제1항 및 제9조제1항에 따라 근로자ㆍ공무원 및 교직원이 다음 각 호의 어느 하나에 해당하는 경우에는 별지 제6호서식의 직장가입자 자격취득 신고서를 공단에 제출해야 한다. 이 경우 제2조제1항 각 호의 요건을 갖추었는지 여부를 주민등록표 등본으로 확인할 수 없을 때에는 가족관계등록부의 증명서 1부를 첨부해야 한다. <개정 2018.12.18>
