사행행위 등 규제 및 처벌 특례법 시행규칙 제4조(사행행위영업의 허가사항변경)

①법 제4조제2항 및 영 제2조의 규정에 의한 변경허가를 받고자 하는 자는 별지 제5호서식의 변경허가신청서에 그 변경내용을 증명할 수 있는 서류와 허가증을 첨부하여 허가관청에 제출하여야 한다.

②허가관청은 변경허가를 하는 때에는 허가증발급대장에 이를 등재한 후 허가증을 변경기록하거나 재교부해야 한다. <개정 2021.12.31>
